Before you register with the practice – please check our Practice Area Map to make sure you are in our practice area.
The easiest way to register with the practice is using the NHS UK website here.
If you struggle to fill complete the online form you can download the PRF1 registration form here and return the completed form to the practice.
A separate registration form must be filled out each individual person.
We aim to have your registration with the practice complete within 3 working days.
Essential information we require:
- Name and address of your last doctor
- The address at which you were registered with that doctor
- If you are from overseas, your place of birth and the date you arrived in the UK
- Your signature
Every patient is now allocated a named GP. This may be different from the GP you usually see. This does not change the way you book appointments. It means that one GP will have overall responsibility for your care at the surgery. The practice will make reasonable efforts to accommodate any requests regarding any preferences to be with a specific GP. Should you wish to know your named GP please contact Reception.
We recommend all patients sign up to online services. If you wish to do this, wherever possible please provide a mobile number so the practice can text you with your username and password.
Find your NHS number
As a new patient, you are invited to have a registration health check. Please make an appointment with one of our practice nurses. You will need to bring a urine sample with you to this appointment.
